Medical Release Forms (we prefer notarized ones)
Travel Permit if applicable for teams from outside Iowa (this is on thetournamentcenter.com)
Guest Player form if applicable (this is on thetournamentcenter.com)
Please ensure that you have all of the above documentation available when checking in. If any paperwork is missing, then it must be provided by one hour prior to the first game. All local teams must check in on Fridayevening, as there will not be sufficient staffing to deal with large numbers of teams checking in on Saturday. No player can be added to a team once their first game has started.
Out of town teams that cannot check in on Friday evening may check their teams in at the Headquarters tent at Cownie Soccer Complex up to one hour before the first game or before noon whichever one is first on Saturday.
Having your player cards and medical release forms in alphabetical order will really help to expedite the check in process

US Youth Soccer Teams
- US Youth Soccer or USSF player cards for all players.
- Certified state roster.
- Guest player forms as required by US Youth Soccer state associations.
- Medical waiver forms for all players must be shown at registration. However, they will be retained by your team & kept field side.
- Travel papers for out of state teams.
US Club Soccer Teams
- US Club Soccer player cards for all players.
- Official US Club Soccer roster is required (and all guest players must be shown on the official roster).
- Medical waiver forms for all players must be shown at registration. However, they will be retained by your team & kept field side.
Guest Players: maximum of 3 for 8v8, and 4 for 11v11
